Acknowledgment of Receipt of Permit Request (Form T-2)

     This Form acknowledges receipt of your request for a permit to use M-NCCPC fields for a soccer/
softball/baseball (circle one) tournament. All field permit fees and a security deposit in the amount
of ________________ must be received by 4:30 pm on ______________ , or your request will
expire.
The security deposit may be in the form of a cashier's check, money order, or Mastercard or
Visa payment. Your permit application will not receive final approval until permit staff has reviewed it
and received the deposit amount noted above. The deposit will be refunded if the permit is denied. If
your application is approved, the commission will refund the deposit after the tournament in accordance
the terms of this form, the permit rules and regulations, and the permit addendum.

     The applicant agrees to abide by the Deptartment of Parks Rules and Regulations, and by terms of
the permit.


     Commission Park Police review each application to address public safety needs. If necessary, the Commission may take any or all of the following measures to address public health and safety concerns:

a. Require applicant to provide additional toilet facilities (i.e. portable bathrooms) and/or garbage cans.

b. Require applicant to provide, at own cost, additional security. Applicant must first hire off-duty Park
Police Officers if supplemental security is necessary.Applicant may supplement police protection with
securityfrom a private security company licensed by the Maryland State Police. If a possible threat to
public safety is determined, the Director of Parks may deny the permit.

c. The applicant will be notified of the total amount of field permit fees due following approval of his application.

d. The applicant will be required to post a security deposit od $200 per field, not to exceed a total of
$1500 per tournament. The commission may use or any or all of the deposit to pay for excessive clean
up or repairs to any damaged fields. The commission also deserves the right to pursue legal remedies to collect the cost of cleanup or repairs in excess of the security deposit.

e. A commission representative will inspect the permitted fileds during regular business hours (Mon - Fri)
to confirm the pre-tournament condition of fields. The permit holder or designated representative may
attend the inspection.* The representative will inspect the fields again during the first regular business
day after the tournament(the permit holder or designee may be present). Wihtin three weeks following
the tournament, the commission will refund the security deposit or send written notification of damage /cleanup costs and refund any remaining balance.
