Addendum to Permit Regulations (Form T-4)

   The applicant, _____________________________, agrees to the following terms:

1. The applicant will provide additional 30gallon garbage cans for use by all players and fans during the
    tournament period. Applicant guarantees that the grounds will be free of litter at the end of each day,
    and will be responsible for removing the trash collected during the tournament. The applicant will also
    guarantee that the cans will be removed with-in 24 hours after the end of the tournament(as specified
    on the permit). The commission may reatin a reasonable amount of the security deposit necessary to
    remove garbage and/or cans left on the permitted area in violation of this requirement.

2. The applicant will provide additional self-contained portable bathrooms for use by all players and fans
    during the tournament period. The applicant will guarantee removal of the bathrooms with 24 hours
    after the end of tournament. The commission may retain a reasonable amount of the security deposit
    necessary to remove bathrooms left on the permitted area in violation of this requirement.


3. The applicant will comply wih any ohter stated requirements as deemed necessary by the Commission.

4. The applicant will not allow any play on the permitted field when one or more of the following conditions
    exist:

    * Water is standing on the field.
    * One-half inch or more of rain has fallen within the previous 24 hours.
    * The soil is frozen.
    * Steady rain is falling.
    * Bare areas are muddy.
    * The ground feels "spongy".

      Play may not resume until none of these conditions exist. The commission may retain an appropriate
      amount of security deposit necessary to repair any field damage sustained as a result of tournament       playing during any of above conditions.

NOTE: Failure to comply with any of the permit requirements is cause for intermediate revocation of the permit for the duration of the tournament.
